The Position

As a Practice Nurse at Deloraine Medical Centre, you will play a vital role in delivering high-quality care to patients, with a specific focus on supporting individuals with chronic diseases. You will collaborate closely with your fellow healthcare professionals to develop and implement personalised care plans that provide patients with the tools they need to effectively manage their conditions.

Key responsibilities:

• Conduct comprehensive assessments of patients with chronic diseases to gather relevant medical history and assess their current health status.
• Collaborate with healthcare professionals to create individualised care plans tailored to the unique needs of each patient.
• Monitor and track patients' progress, offering ongoing support and guidance to ensure adherence to treatment plans.
• Coordinate and facilitate regular follow-up appointments to assess the effectiveness of interventions and make necessary adjustments to care plans.
• Maintain accurate and up-to-date medical records, thoroughly and accurately documenting patient interactions.
• Assume treatment room responsibilities, including wound care and dressings, administration of immunisations, assistance with minor procedures, patient education, and triage.

To excel in this role, you should possess:

• Current AHPRA registration as a registered nurse
• Exceptional interpersonal and communication skills
• Proficiency in using electronic health records and other medical software.
• Ability to work collaboratively in a fast-paced and team-oriented environment.
