An exciting opportunity exists for a full time social worker to join a leading multidisciplinary team in the stunning Huon Valley.

The Position

This is a full time position for a social worker to join the team and the role is funded by Primary Health Tasmania for a fixed term until February 2022.

The position was initially created as a response to the 2019 bushfires and now also has a focus on the Covid-19 pandemic. The role will focus on reducing symptoms and improving the quality of life for patients and ensure people have a positive experience accessing the health service system.

To be successful you must have demonstrated mental health and case management experience and have a strong commitment to providing best practice, person focused care to all patients. As an added bonus you will get to work across multiple locations in the beautiful Huon Valley and also Bruny Island. You will be able to utilise a mix of video conferencing and home visits to connect with people.

A tertiary qualification in social work is essential, membership or eligibility for membership with AASW is also a requirement.

The Organisation

The Huon Valley Health Centre is a teaching medical practice that values learning and seeks to overcome health inequalities by offering accessible, affordable and highly evidence based medical care to their community.

The team consists of a number of general practitioners, nurses and allied health professionals working across their 3 locations in Huonville, Cygnet and their Mary Street Clinic.

The salary will be negotiated at the Health Professionals and Support Services Award 2010- Health Professionals level 2.

The team values professional development and if successful you will also be eligible to apply for a Health Workforce Scholarship Program through HR+ Tasmania. This is for up to the value of $10,000 per year over 2 years to complete post graduate studies or other identified professional development.
