Flinders Island Aboriginal Association is seeking to recruit a motivated health professional to the role of Chronic Disease and Aged Care Coordinator.
With a strong focus on client centred care, the Care Coordinator works collaboratively with Aboriginal and Torres Strait Islander people in the community to achieve positive health outcomes, support clients to better self-manage chronic conditions and improve access to healthcare.
The role is perfect for a health professional committed to the provision of coordinated and holistic service delivery that encompasses the physical, emotional, spiritual and environmental aspects of health.
The Flinders Island Aboriginal Association Incorporated (FIAAI) is and Aboriginal community controlled organisation established in 1971 by a group of local Aboriginal people and their partners.
FIAAI provides a range of services on Flinders Island including: Primary Health and Aged Care, Housing, Youth Services, a State-wide Tackling Smoking Project and three business enterprises.
